What Is Exposure and Response Prevention (ERP) Therapy for OCD? - By Kenedee Parry, LMSW

If you live with obsessive-compulsive disorder (OCD), you may feel caught in a cycle of unwanted thoughts, anxiety, and behaviors you feel compelled to do to find relief. You might check, avoid certain situations, seek reassurance, mentally review memories, or repeat actions until they feel “just right.”

Exposure and Response Prevention (ERP) is a specialized form of cognitive behavioral therapy (CBT) and a leading treatment for OCD. ERP therapy helps you change how you respond to obsessive thoughts, reduce compulsive behaviors, and build confidence in handling uncertainty.

How Does ERP Therapy Work?

ERP involves gradually facing situations, thoughts, or feelings that trigger anxiety or uncertainty (exposure) while practicing not engaging in the compulsive behaviors that usually follow (response prevention).

For example, someone with contamination-related OCD might gradually practice touching something they consider “contaminated” while resisting the urge to immediately wash their hands.

The goal isn't to make anxiety disappear instantly. Instead, ERP helps you learn that anxiety and uncertainty can be uncomfortable without requiring you to perform a compulsion. Over time, this can weaken the OCD cycle and create more freedom in everyday life.

Does ERP Mean Facing Your Biggest Fear?

No. Effective ERP therapy is gradual, collaborative, and individualized. A therapist helps you develop exposures that feel challenging but manageable while supporting you throughout the process.

ERP can also address less obvious compulsions, including reassurance seeking, excessive researching or Googling, mental checking, reviewing memories, and trying to determine what an unwanted thought means.

Is ERP an Effective Treatment for OCD?

ERP is considered a first-line psychological treatment for obsessive-compulsive disorder. Although the process can feel challenging, many people find that learning to respond differently to obsessive thoughts helps them experience greater flexibility and freedom in their daily lives.

You don't have to wait until OCD feels unbearable to seek help. If intrusive thoughts, compulsions, or avoidance are affecting your relationships, school, work, or everyday routines, OCD therapy may be a helpful next step.
